CMRO  is the flagship medical journal. An open access, open peer-reviewed general medical journal,  CMRO outstanding and influential research in all areas of clinical practice, translational medicine, medical and health advances, public health, global health, policy, and general topics of interest to the biomedical and sociomedical professional communities. We also publish stimulating debates and reviews as well as unique forum articles and concise tutorials.

Telemedicine and In-Person Visit Modality Mix and Electronic Health Record Use in Primary Care

Telemedicine use increased substantially during and after the COVID-19 pandemic 1 and has the potential to provide low-acuity medical services at lower costs. 2 However, telemedicine also levies new costs on clinicians. 3 Telemedicine requires shifting care delivery workflows, as it rarely includes clinical support staff but can involve levels of patient complexity similar to in-person visits. 4 , 5 This may increase administrative and electronic health record (EHR) burden for clinicians and increase cognitive costs as clinicians switch modalities. In a recent study, 6 greater weekly telemedicine visit share was associated with increased EHR time, including after-hours time, mostly spent in documentation. Our study aimed to address 2 gaps: first, whether day-to-day changes in telemedicine share demonstrate a similar association with EHR time; and second, what changes occur in domains of EHR use not examined in previous studies (eg, medical record review, orders).

This cross-sectional study combined visit modality data with EHR active use data capturing time spent by primary care physicians (PCPs) in the Cerner EHR system from December 2021 through June 2023 at MedStar Health, a large multispecialty health system in the mid-Atlantic region. We calculated PCPs’ daily telemedicine share as the percentage of the day’s visits conducted via telemedicine and categorized this variable into 5 levels. Because we used deidentified data, this study was deemed exempt and not human participant research by the Georgetown University–MedStar Health Institutional Review Board; we followed the STROBE reporting guideline.

Telemedicine visits were identified via registration and scheduling records. We analyzed 5 measures of active EHR time for each PCP-day: total EHR time, documentation time, medical record review time, order time, and next-day documentation time (only for PCP-days with a consecutive qualifying PCP-day). We calculated descriptive statistics and ran ordinary least squares linear regression models, adjusting for visit volume and physician and calendar-day fixed effects. These models estimate the marginal within-clinician association between each telemedicine share level and our outcomes relative to zero-telemedicine days while adjusting for common temporal trends. We used R statistical software, version 3.6.3 (R Project for Statistical Computing) (tidyverse, fixest packages) for analyses, using 2-tailed hypothesis tests (α = .05).

The study included 316 PCPs observed across 67 894 PCP-day observations distributed across 5 daily telemedicine share categories (zero daily telemedicine share, 44.7% of all PCP-days; ≤10% share, 17.2%; 11%-25% share, 24.8%; 26%-99% share, 11.1%; and 100% share, 2.2%); mean (SD) overall visit volume, 13.9 (7.2) visits/d ( Table ). All outcomes demonstrated statistically significant differences across telemedicine share levels. The mean (SD) documentation time for PCPs was 71.3 (54.3) minutes on zero-telemedicine days and 87.1 (50.0) minutes on days with up to 10% telemedicine visits. In regression analyses, days with a mix of visit modalities were associated with significantly greater time for EHR, documentation, and medical record review ( Figure ). Compared with zero-telemedicine days, 26% to 99% telemedicine days were associated with 14.8 (95% CI, 7.6-22.0) more minutes of active EHR time (5.6% increase, P  < .001), 4.7 (95% CI, 1.2-8.3) additional documentation minutes (6.0% increase, P  = .01), and 5.5 (95% CI, 2.8-8.2) additional medical record review minutes (6.2% increase, P  < .001). Telemedicine share resulted in a negligible increase in order time and had no association with next-day documentation time ( Figure ).

This cross-sectional study found that, during clinic days with both telemedicine and in-person visits, PCPs had 5.6% to 6.2% more EHR-based work. This work did not spill over into next-day documentation, suggesting that PCPs absorbed added time into their workload on mixed-modality days. However, we found that fully telemedicine days were not associated with EHR-based work, contrary to previous findings. 6 We attribute this difference to the small sample of fully telemedicine PCP-days in our study in comparison with prior work 6 (2.2% vs 16.5% of physician-weeks) as well as higher mean visit volume (13.9 visits/d vs 20 visits/wk) ( Table ). Greater EHR time may be due to increased multitasking during telemedicine visits, as PCPs simultaneously engage with patients and the EHR during telemedicine visits in ways that are not possible in person. This multitasking may feel more efficient and therefore may not register as “burdensome”; further research should explore whether added EHR time associated with mixed-modality days further burdens PCPs. Limitations of our study include our setting of a single health system, lack of information on visit and patient characteristics and on clinicians’ experience with telehealth tools, and lack of clinical outcomes.

The African Journal of Current Medical Research (AfriJCMR), an international peer reviewed online medical journal was started in January 2016 by the African Current Medical Research Limited. We are primarily set up in Kumasi, Ghana but have collaborators in other African countries, Europe and the United States.

New Terms to Describe the Steatotic Liver

Experts have retired “nonalcoholic fatty liver disease” in favor of a roster of “metabolic dysfunction–associated” alternatives.

Medical terminology evolves just as all languages do, with new terms replacing older ones as scientific understanding, clinical practice, or cultural norms change. All three considerations have led to a paradigm shift in the language of hepatology, as an international panel endorsed replacing “nonalcoholic fatty liver disease” (NAFLD) with “metabolic dysfunction–associated steatotic liver disease” (MASLD), creating a downstream cascade of new diagnostic entities ( Hepatology 2023 Dec; 78:1966 ; Aliment Pharmacol Ther 2024 Jan; 59:150 ).

MASLD has become not only the most common chronic liver disease in the world but also one of the most common chronic diseases overall, affecting an estimated 30% to 40% of the world's population ( J Hepatol 2023 Sep; 79:842 ). That fact alone contradicts the implication that NAFLD refers to an uncommon diagnosis of exclusion. Furthermore, both “alcoholic” and “fatty” have overtones of censure that modern medical language tries to avoid. Meanwhile, older diagnostic criteria assumed that most liver pathology resulted from a single cause, but clinicians often encounter multiple liver insults in the same patient.

So the fatty liver has become the steatotic liver, and “steatotic liver disease” (SLD) is now the preferred umbrella term for MASLD and ALD (alcohol-related liver disease), as well as for cases of steatosis caused by drugs and genetic factors, and for cases without clear etiology (cryptogenic SLD). A diagnosis of MASLD requires the presence of only a single metabolic risk factor on a list that includes hyperglycemia, overweight, and other hallmarks of the metabolic syndrome. Other causes of steatosis must be excluded, but notably, a diagnosis of MASLD does not require the absence of viral hepatitis, as a diagnosis of NAFLD did. Nor does a diagnosis of MASLD require the absence of alcohol use: Patients with MASLD who drink more alcohol than the recommended maximum (20–30 g daily) but less than the threshold for diagnosis of ALD (50–60 g daily) now will be said to have MetALD (MASLD and increased alcohol consumption).

Among patients with inflammatory steatosis, MASH (metabolic dysfunction–associated steatohepatitis) will replace NASH (nonalcoholic steatohepatitis). Those with nonprogressive disease at the other end of the clinical spectrum will be said to have MASL (metabolic dysfunction–associated liver) rather than NAFL (nonalcoholic fatty liver). Meanwhile, the recently coined term MAFLD (metabolic dysfunction–associated fatty liver disease) will be retired, with most of those patients folded into the MASLD category.

Clinicians, researchers, and medical publications inevitably will need some time to readjust their language and protocols around the new scheme. (In recent NEJM Journal Watch summaries, we have used both old and new terms, reflecting their use in the corresponding original articles.) Further, difficult follow-up questions must be addressed: How applicable are older research studies to patients who now are categorized somewhat differently? Are all metabolic risks really equivalent? Should patients' notoriously unreliable estimates of their own alcohol consumption really be analyzed so precisely? But at the end of the transition, the reward will be a set of diagnostic entities that more closely reflects modern clinical realities.

Pirogov Medical University offers a healthy student life and an opportunity to experience life in Moscow, the capital city of Russia, and also the most vibrant and exciting location in the largest country in the world! 

For affordable accommodation of students and make their living experience safe and better, the University maintains a comfortable dormitory. Every room is shared by two or three students, and each floor has a shared kitchen where students can cook their food. Members of the dormitory help the newcomers to settle in their new homes. For the safety of the students, the University’s security team maintains 24-hour surveillance and is capable of providing emergency response, if required.

To help students adjust to life at university, it has a dedicated Student Support System in place. Every group of new students is assigned to two professors who guide the students not only about studying but about living as well, helping students adjust to the new environment and feel comfortable.

To keep students fit and active, Sports Center on the campus is equipped with facilities to play various sports, including badminton, basketball, volleyball, table tennis, swimming, football, hockey, chess, etc. Student can unleash their creativity by indulging in extracurricular adventures offered by Student Organizations and Societies. On the campus, there are ample opportunities for self-improvement and taking part in music, dance, sports competitions, and theater. 

For peer support, the University has a Student Council in place which offers support in academic as well as non-academic matters making student life stress free.

For the social upliftment and help students connect with the society and local people, they are involved in community and welfare organized by the University, including medical outreach, health awareness programs, and blood donation camps.  The University Volunteer Center organizes a number of volunteer activities to help students contribute to social causes.

Living in Moscow, students can explore its cultural heritage, museums, historic buildings, the world-famous Alexander Garden, and much more. For traveling in Moscow, students don’t face any problems, thanks to its convenient and cheap transportation system, especially the Moscow Metro.

With the charm of Moscow and all the student facilities and support services offered by the University, student life at Pirogov Medical University is a delight.
