Evidence-based research in pediatric nutrition

This is an interesting book with three distinct sections, “Methodological Considerations”, “Impact of Early Nutrition on Later Outcomes”, and “Issues in Infant Feeding”. In the first section, there is extensive discussion on what a simple review is versus a systematic review and a meta-analysis. There is an overview of methods for conducting observational studies as well as human intervention trials in nutrition and the strengths and weaknesses of evidence generated by them. It has been put forth that both are important. While the clinical trials can give evidence on effectiveness, the observational studies, especially those with prolonged follow up, can assess long-term safety, hence both are pertinent in paediatric nutrition arena and complement each other. A section has been devoted on designing, analysing and interpreting the results of clinical trials. The importance of meta-analysis has been put forth along with the importance of forest plots in understanding the effect of heterogeneity due to clinical as well as methodological issues. There is a note of caution while conducting trials with vulnerable population of young infants and pregnant women.

In the second section, discussions are revolved around evidence generated by animal and human nutrition studies. In addition, Barker's hypothesis has been explained which promulgates that foetal malnutrition resulting in lower birth weight is associated with increasing mortality due to ischaemic heart disease in adults. The short as well as long term benefits of breastfeeding have been enumerated along with the risks associated with accelerated growth. Evidence has been put forth for short as well as long term risks of undernutrition like propensity to infections and lower cognition. The importance of iron and long-chain polyunsaturated fatty acids has also been discussed.

In the third section there are nine chapters, covering most of the contemporary issues and myths. The value of probiotics, prebiotics and dietary fibre in the management of functional intestinal disorders has been discussed. Based on the evidence available, the possible role of specific probiotics in infantile colic and constipation has been highlighted.

A relatively new type of food product is being marketed as Growing-up milk (GUM). This concept is most prevalent in developed countries and is relatively new in the Indian scenario. GUM products are not a necessity and are being marketed for compensating the nutritional deficiencies, which may occur during transition from infant nutrition to family food. These products are commercially available, fortified, and nutritionally balanced but can lead to disinterest in food on part of the infant and feeding on part of the parent.

The book has lucidly continued the debate on relative merits and demerits of cow's and goat's milk with the latter having no clear advantage over the former. It has put forth evidence to support that cow's milk should be given along with complementary foods and not alone. In the next chapter the paucity of evidence on the relationship of the timing of introduction of complementary foods to the risk of disease in later life has been discussed.

A chapter is devoted to the discussion of evidence for dietary prevention of primary allergy. It is stated that a large proportion of allergic disorders in children have genetic basis. There is, however, inconsistent data on prevention of primary allergy with breastfeeding. If breastfeeding is not possible, it is relatively safe to use cow's milk protein hydrolysate infant formulae. It is discussed that there is no evidence for any dietary restriction during pregnancy.

Another chapter is on the mineral (iodine, iron and calcium) and vitamin (B12 and Vitamin D) supplementation and states that none of the infant milk formulae have recommended amounts of these. Also, perhaps the recommended dietary allowances are an overestimation of the real requirements in infants. This chapter is adequate only for the nutritionists, however, for paediatricians, further information on preterm supplementation would have been beneficial. The next chapter is devoted to strategies for the prevention of celiac disease through early infant nutrition. The most critical issue revolves around the correct timing of introduction of gluten in the complementary feeding schedule and protective effect of breastfeeding when gluten is introduced.

There is a global endemic of obesity in children and the last chapter has taken up this issue very aptly and succinctly. In children, during diet therapy for obesity, care has to be taken to ensure adequate growth and development with the target to only cause fat loss. This has to be combined with psychological interventions and lifestyle modifications. There is evidence supporting drug therapy also. Hence the fight against childhood obesity is multi-component and a multi-pronged approach is hence required.

Overall, the book has comprehensive and recent information and is of scientific value. It is easy to read and understand. Tables and diagrams further add to the understanding. Each chapter puts forth scientific evidence for or against a topic in an unbiased manner and does not end up simply with the writing of recommendations and guidelines. This book will benefit postgraduates and practitioners of paediatrics and paediatric nutrition.

Research team creates a chemistry map for human cells

Scientists shed light on the chemistry of human cells

Scientists at NPL worked with Diamond Light Source to publish a study that shows how the chemistry of human cells changes, depending on the structure of their extracellular niche, are major determinants of cell responses and development pathways. The paper is published in ACS Applied Materials & Interfaces .

The human body undergoes renewal by specializing "blank" cells—termed stem cells—into primary cells organized into tissues according to their environment. The environment is created by extracellular matrices or scaffolds on which cells build up tissues and organs.

Cell responses to these matrices provide performance metrics that are critical for the development of cell-based diagnostics and therapies. Some of the important metrics are obtained using microscopy and biological assays, which, however, fail to capture the chemistry of cells and how it changes at different cell-matrix interfaces.

This shortcoming continues to hamper progress in health care and technology innovation , as chemistry is the direct reflection of cellular processes responsible for tissue growth and repair.

Scientists at NPL set out to address this gap by recording InfraRed maps of human primary and stem cells grown on native and synthetic matrices.

InfraRed spectroscopy can access virtually all chemical information available in the cell but cannot tell the cell from its matrix or recognize different parts of the same cell. Therefore, a correlative approach involving the use of physical imaging, provided by light and atomic force microscopy , to guide chemical spectra was necessary.

To achieve this, NPL collaborated with beamline scientists at Diamond Light Source, biologists from Sheffield and London Colleges and data expertise from Cambridge. Together, they developed a spectral imaging approach which not only allowed them to obtain chemical maps of single cells but also to cross-compare their chemistry signatures in response to matrices exhibiting distinct physical properties.

Their study also demonstrates the efficiency of correlative measurements to explain cell behavior at cell-matrix interfaces in 2D, as well as the need to develop analogous and more advanced methodologies to measure cell-matrix interfaces in 3D, pathing a way to impact for health care and solutions towards human tissue regeneration.

Max Ryadnov, NPL Fellow, said, "The study was an exciting collaboration which provided us with important insights into how the chemical make-up of human cells correlates with the physical changes of the molecular scaffolds that support their growth and development. The study also informed next steps in our developments focusing on correlative measurements of living biological systems."

Deposit Insurance, Uninsured Depositors, and Liquidity Risk During Panics

The lack of universal deposit insurance coverage can create liquidity risk during financial crises. This aspect of deposit insurance is hard to test in modern data because of the broad coverage of most systems. We, therefore, study the role that the U.S. Postal Savings System played in commercial bank closures during the Great Depression. The system offered households a federally insured deposit account at post offices throughout the nation, and its structure provides a near-ideal environment to identify this competitive liquidity risk during a crisis. We find that banks that operated nearby a post office that accepted deposits were more likely to close between 1929 and 1935. We further make use of a structural change in the availability of postal depositories in the early 1910 to estimate an IV regression that confirms the results. In either model, the effect is strongest for those banks with low reserves, suggesting that the mechanism was through depositor withdrawals rather than other factors.

Talking politics with strangers isn't as awful as you'd expect, research suggests

Many of us avoid discussing politics with someone who holds an opposing viewpoint, assuming the exchange will turn nasty or awkward. But having those conversations is far more gratifying than we expect, a new research paper suggests.

Across a series of experiments involving hundreds of U.S. adults, a team of scientists found that individuals underestimate the social connection they can make with a stranger who disagrees with them. The findings are published in Psychological Science, a journal of the Association for Psychological Science.

These low expectations may help to explain why people think those on the opposite side of the political spectrum have more extreme views than they actually do, behavioral scientists Kristina A. Wald (University of Pennsylvania), Michael Kardas (Oklahoma State University), and Nicholas Epley (University of Chicago) wrote in an article about their research.

"Mistakenly fearing a negative interaction may create misplaced partisan divides," they wrote, "not only keeping people from connecting with each other but also keeping people from learning about each other and from each other."

The experimenters found evidence, through experiments conducted online and in person, that people prefer to avoid hot-button issues, especially with people who disagree with them. People also tend to advise their friends and relatives to avoid such conversations.

But Wald, Kardas, and Epley believed people would find discussing their political differences to be a more positive experience than expected, at least partly because people fail to appreciate the extent to which conversations are informative and draw people closer together.

To test their theory, they asked nearly 200 participants in one experiment for their opinions on divisive political and religious topics, such as abortion and climate change. The researchers then divided the participants into pairs and assigned them to discuss one of these topics. Some participants were told in advance whether their partners agreed with them or not, but others entered the discussions unaware of their partners' views.

All the participants reported how positively or negatively they expected the conversation to be, then engaged in the discussion while being video recorded. Afterward, the participants rated their sentiments about the dialogue. Research assistants also viewed the videos of the conversations and evaluated them across several dimensions.

As predicted, the participants underestimated how positive their conversation experience would be, but this tendency was largest when they disagreed with their partner. Participants in this disagreement condition also underestimated the similarities in their opinions. Coders who watched the videos of these conversations confirmed that participants tended to stay on topic, and that the conversations were consistently positive whether the participants agreed or disagreed.

In another experiment, the researchers tested their hypothesis that people underestimate how the process of conversation itself -- actual back-and-forth dialogue -- connects people. To do so, they randomly assigned participants to discuss a divisive topic they agreed or disagreed on, but they also randomly assigned participants to either have a conversation about the topic in a dialogue format or to simply learn of their partners' beliefs on the topic in a monologue format. In the monologue format, each person separately recorded themselves talking about their opinion and then watched the other person's recording.

Overall, the participants underestimated how positive their interactions would be, especially when they disagreed with their partner, the researchers noted. But this tendency was especially strong when people actually had a conversation with their partner rather than simply learning of their beliefs in a monologue. The social forces in conversation that draw people together through back-and-forth dialogue are not only powerful, but they appear to be even more powerful than people expect.

The researchers cautioned that their experiments involved participants talking with strangers; the experiments did not reveal how disagreements unfold among family and friends. Still, they said their findings illustrate the benefits of talking and listening to others rather than typing and broadcasting in debates on social media.

Our reluctance to discuss our differences denies us some positive social interactions, the authors concluded.

"Misunderstanding the outcomes of a conversation," they wrote, "could lead people to avoid discussing disagreements more often, creating a misplaced barrier to learning, social connection, free inquiry, and free expression."

The funding will drive innovation in equitable clean transportation and is aligned with strategies detailed in the U.S. National Blueprint for Transportation Decarbonization . 

The funding is through DOE’s Office of Energy Efficiency and Renewable Energy (EERE). Topic areas in the Vehicle Technologies Office (VTO) Fiscal Year (FY) 2024 R&D funding opportunity include:

  • Next-generation phosphate-based cathodes.
  • Advancing the state of the art for sodium-ion batteries.
  • Developing concepts for decreasing greenhouse gas emissions from off-road vehicles such as construction, agriculture, mining, and forestry vehicles.
  • Developing and deploying vehicle-to-everything technologies that can lead to meaningful savings at the vehicle and transportation system level.
  • Developing high-performance, domestically produced electrical steels (E-steels) for use in electrified powertrains.
  • Addressing critical cybersecurity needs for smart and secure electric vehicle charging.

As part of the Biden-Harris Administration’s commitment to ensuring the benefits of a clean transportation system are shared equally, the funding seeks the participation of underserved communities and underrepresented groups. Applicants are required to describe how diversity, equity, and inclusion objectives will be incorporated into their project.

Topic Areas

Topic Area 1: Next-Generation Phosphate-Based Cathodes

This topic area targets the development of phosphate-based cathode materials that surpass the performance of state-of-the-art lithium iron phosphate (LFP) cathode materials, which are currently gaining traction as an alternative low-cost solution. The primary objective of this area of interest is to develop high energy density battery cells containing phosphate-based cathodes at the material and cell level.

Topic Area 2: Na-ion Battery Seedling Projects for Electric Vehicle Applications

While shifting to alternative cathode materials like LFP can alleviate the impact of nickel and cobalt, the impact of lithium has not been adequately addressed. One alternative to lithium is sodium (Na). While there is much promise for Na-ion chemistries, key issues still limit their adoption. This objective of this topic area is to advance the state of the art for Na-ion batteries by solving key challenges for the cathode, anode, or electrolyte through the development of 1 Ah full cells utilizing cell chemistries that are significant advancements over current industry state-of-the-art Na-ion technology.

Topic Area 3: Low-GHG Concepts for Off-Road Vehicles

The objective of this topic area is to develop and validate technology concepts capable of significantly decreasing greenhouse gas emissions, energy use, harmful criteria emissions, and total cost of ownership across the entire off-road vehicle sector, including construction, agriculture, mining, forestry, ports, warehouses, etc. Concepts must demonstrate they can meet the unique requirements for off-road vehicles and gain customer acceptance.

Topic Area 4: Saving Energy with Connectivity

Research has shown that vehicle-to-everything (V2X) communications can lead to meaningful energy savings at the vehicle and transportation system level by integrating interoperable vehicle-to-vehicle (V2V), vehicle-to-infrastructure (V2I), and vehicle-to-pedestrian (V2P) communications. The objective of this topic area is to develop and deploy V2X technologies with a focus on the efficiency and convenience of the mobility ecosystem, while reducing transportation’s environmental impacts. Examples could include but are not limited to eco-driving along connected corridors, transit or freight priority, integrated corridor management, or passenger or freight trip-chaining optimization.

Topic Area 5: Domestically Produced Electrical Steels (E-Steels)

The US transportation sector is in a technology revolution where light-duty vehicles are rapidly transitioning from internal combustion engines to electrified powertrains. Although most of the vehicles are produced in the US, many of the powertrain components rely on imports and foreign supply chains. Of particular interest are traction motors and their components. The objective of this topic are is to develop E-Steels meeting properties including frequency, thickness, ductility, cost, and manufacturability. 

Topic Area 6: Cybersecurity for Smart and Secure Electric Vehicle Charging

This topic area is addressing critical cybersecurity needs to address through two subtopics: 

  • Subtopic 6.a: Enabling Wide-scale, Cybersecure EV/EVSE Aggregation for Grid Services :  To support the integration of electric vehicles (EVs) and their charging requirements with the electric grid, both government and the private sector have made significant investments in the development of smart charge management (SCM) systems and technologies for EV charging infrastructure. The objective of this subtopic area is to research, develop, and demonstrate systems, technologies, and tools necessary for the cybersecure aggregation of EVs and charging infrastructure to provide widescale, cybersecure grid services.
  • Subtopic 6.b: Tools to Assess EV/EVSE/Charging System Cybersecurity Posture and Compliance with Standards and Protocols for Communications, Controls, and Monitoring :   Testing and evaluation of Electric Vehicle Supply Equipment (EVSE) by DOE national laboratories has clearly indicated a lack of compliance by many vendors with certified and/or regulated EV charging standards and protocols. In addition to creating cybersecurity vulnerabilities, this non-compliance greatly inhibits interoperability, supplier-managed SCM, and right-to-repair. The objective of this subtopic is to research, develop, and validate a suite of tools and associated procedures to comprehensively assess EV/EVSE/charging system compliance with relevant standards and protocols and cybersecurity posture.

Solar eclipse glasses are needed for safety, but they sure are confusing. What to know.

You can't control the weather or the traffic , but one thing you can do right now ahead of the total solar eclipse is to get yourself a pair of special eclipse glasses , so you can safely view the cosmic spectacle.

The total eclipse will cross over a dozen states as it traverses from Texas to Maine on the afternoon of April 8.  Millions of people are expected to travel  to see it.

Astronomers and other experts say that eclipse watchers should wear  specialized eyewear  crafted to  protect your sensitive retinas  from permanent damage. And that means eclipse glasses .

Fortunately, solar eclipse glasses are still readily available  from plenty of vendors across the internet. Unfortunately, there's fears of fake glasses , and sometimes testing your glasses at home is the best way to know if they're safe.

Here's  what to know  about eclipse glasses and where you can get a pair of your own.

Where to find solar eclipse glasses

As the eclipse approaches, many places like local libraries, astronomical observatories, schools and other public agencies are providing proper eyewear, often for free.

Or you can just order them on the internet: Many eclipse websites like GreatAmericanEclipse.com and NationalEclipse.com sell a variety of eyewear products, along with plenty of other gadgets one may need to see the eclipse.

Of course,  Amazon  and plenty of other online retailers are also selling solar eclipse glasses in bulk, meaning you'll have plenty to share with your friends and family. American Paper Optics, the nation's largest supplier of eclipse glasses, even offers  express shipping  across the country.

Watch out for fake glasses

With online platforms like Amazon listing thousands of eclipse viewers, fears of fake or counterfeit glasses have grown.

"We do see glasses coming from China that have printing on them that say they're made in the U.S. or that they are made by one of the American manufacturers," Rick Fienberg, the project manager of the American Astronomical Society's solar eclipse task force, told USA TODAY.

When in doubt, buyers should refer back to the list of  official sellers and manufacturers maintained by the AAS , which communicates directly with sellers and manufacturers and checks test reports.

The list includes well-known North American manufacturers like American Paper Optics, as well as several large grocery and retail chains in the U.S. like Walmart, Lowe's and Kroger.

Real, but rare, examples of eye damage

Doctors and a rare set of eclipse watchers have warned  about watching the eclipse without adequate eclipse glasses or with the naked eye that has resulted in rare cases of eye damage. The damage occurs because people's natural response to squint when looking at direct sunlight does not get triggered when a partial eclipse happens.

Such was the case with one Staten Island woman who  watched the 2017 eclipse using faulty glasses that caused severe retinal damage. Doctors from Mount Sinai's New York Eye and Ear Infirmary chronicled this luckily rare case in  JAMA Ophthalmology , a medical journal. The woman told doctors that she had  gazed at the moon passing in front of the sun  through what she  thought were protective glasses . The damage on her left retina, the area at the back of the eye where the brain receives images, looked like the shape of a partial  eclipse .

"It's a very focused beam of high-energy light from the sun itself,” Dr. Avnish Deobhakta, an ophthalmologist at the Mount Sinai infirmary who treated the woman, told USA TODAY. “It can actually destroy parts of the retina, and certainly destroy it in the shape of an eclipse.”

How can you test eclipse glasses?

NASA shared an easy method to check eclipse glasses at home.

Buyers should put on their glasses and look at a bright light, like a flashlight. If the light is "extremely dim," or doesn't appear at all, the glasses are safe, Susannah Darling, of NASA's Goddard Space Flight Center, said in an instructional video. Viewers should be able to see the filament of the lightbulb, not the glow surrounding the bulb.

With safety in mind, there are ways to get free solar eclipse glasses .

The online eye retailer Warby Parker is offering members of the public a pair of ISO-certified glasses starting Monday. Warby Parker retail locations nationwide, find one  here.

But there are plenty of other retailers giving out free glasses that range from Smoothie King to the protein bar Perfect Snacks, which will give out glasses alongside a chocolate brownie flavor.
