The role of case studies for students as they learn about the different theories is essential for them to grasp a practical understanding of applications. This book covers the knowledge of writing case reports in a clear and comprehensive way.

John McLeod is and probably always will be the author we recommend when talking about evidence based practice. His other texts in Doing Counselling Research and Qualitative Research are seminal in this context. This text fills the gap around working with case studies in research, and is very relevant as this is an area that most counsellors will need to be familiar with. The book is well written and if possible makes the subject even more approachable and interesting. Our third year students are now recommended to read this title in relation to all of the third year professional level study modules, as it offers so the opportunity to become familiar with research methodologies and language.

An excellent book which helps people understand the ethics and processes involved in a case study approach. Some very useful examples contained within the book which makes the subject understandable. It has provided me with the motivation to consider applying this in my own practice.

Part of the beauty of this book is the accessibility of the author; as he brings the reader through an exciting, interesting, pragmatic and richly informed account of living qualitative research in action. The book considers; pragmatic, n=1, HSCED, theory-building adn one of my personal favourites - narrative approaches. This book is a required resource for anyone interested in qualitative research or therapy practice. A gem.

This is a useful book, that makes a considered case for the the use of Case Studies for effective research as well as a developmental method for students.

Excellent book, which fills a gap in the current literature; especially useful in clinical psychology training where alternatives to n=1 empirical case studies are not widely accepted.

Guidelines To Writing A Clinical Case Report

What is a clinical case report.

A case report is a detailed report of the symptoms, signs, diagnosis, treatment, and follow-up of an individual patient. Case reports usually describe an unusual or novel occurrence and as such, remain one of the cornerstones of medical progress and provide many new ideas in medicine. Some reports contain an extensive review of the relevant literature on the topic. The case report is a rapid short communication between busy clinicians who may not have time or resources to conduct large scale research.

WHAT ARE THE REASONS FOR PUBLISHING A CASE REPORT?

The most common reasons for publishing a case are the following: 1) an unexpected association between diseases or symptoms; 2) an unexpected event in the course observing or treating a patient; 3) findings that shed new light on the possible pathogenesis of a disease or an adverse effect; 4) unique or rare features of a disease; 5) unique therapeutic approaches; variation of anatomical structures.

Most journals publish case reports that deal with one or more of the following:

  • Unusual observations
  • Adverse response to therapies
  • Unusual combination of conditions leading to confusion
  • Illustration of a new theory
  • Question regarding a current theory
  • Personal impact.

STRUCTURE OF A CASE REPORT[ 1 , 2 ]

Different journals have slightly different formats for case reports. It is always a good idea to read some of the target jiurnals case reports to get a general idea of the sequence and format.

In general, all case reports include the following components: an abstract, an introduction, a case, and a discussion. Some journals might require literature review.

The abstract should summarize the case, the problem it addresses, and the message it conveys. Abstracts of case studies are usually very short, preferably not more than 150 words.

Introduction

The introduction gives a brief overview of the problem that the case addresses, citing relevant literature where necessary. The introduction generally ends with a single sentence describing the patient and the basic condition that he or she is suffering from.

This section provides the details of the case in the following order:

  • Patient description
  • Case history
  • Physical examination results
  • Results of pathological tests and other investigations
  • Treatment plan
  • Expected outcome of the treatment plan
  • Actual outcome.

The author should ensure that all the relevant details are included and unnecessary ones excluded.

This is the most important part of the case report; the part that will convince the journal that the case is publication worthy. This section should start by expanding on what has been said in the introduction, focusing on why the case is noteworthy and the problem that it addresses.

This is followed by a summary of the existing literature on the topic. (If the journal specifies a separate section on literature review, it should be added before the Discussion). This part describes the existing theories and research findings on the key issue in the patient's condition. The review should narrow down to the source of confusion or the main challenge in the case.

Finally, the case report should be connected to the existing literature, mentioning the message that the case conveys. The author should explain whether this corroborates with or detracts from current beliefs about the problem and how this evidence can add value to future clinical practice.

A case report ends with a conclusion or with summary points, depending on the journal's specified format. This section should briefly give readers the key points covered in the case report. Here, the author can give suggestions and recommendations to clinicians, teachers, or researchers. Some journals do not want a separate section for the conclusion: it can then be the concluding paragraph of the Discussion section.

Notes on patient consent

Informed consent in an ethical requirement for most studies involving humans, so before you start writing your case report, take a written consent from the patient as all journals require that you provide it at the time of manuscript submission. In case the patient is a minor, parental consent is required. For adults who are unable to consent to investigation or treatment, consent of closest family members is required.

Patient anonymity is also an important requirement. Remember not to disclose any information that might reveal the identity of the patient. You need to be particularly careful with pictures, and ensure that pictures of the affected area do not reveal the identity of the patient.

Long Covid Fibromyalgia  Perspectives from Pain Psychology

Recognizing an increase in fibromyalgia-like symptoms reported by patients after becoming infected with COVID-19 led Dennis Given, PsyD '05, a pain psychologist working in the Fibromyalgia Program at Penn Medicine Lancaster General Health, and Joseph Harrison, MS, MBDS, (PsyD '25), a fourth-year clinical psychology doctoral candidate at PCOM, to collaborate on their case study, “Biopsychosocial Management of Long Covid Fibromyalgia: Lessons from the Penn Medicine Lancaster General Health Fibromyalgia Program.”

The article is published in The Journal for Integrated Primary Care (JIPC) — PCOM's first peer-reviewed scientific journal , which aims to grow the evidence base related to integrated primary care professional education, practice and intervention—and includes a summary of their clinical experiences with patients in the Fibromyalgia Program. It also provides information on how Long Covid may lead to the development of fibromyalgia symptoms that include widespread pain, fatigue, sleep disturbances and impairments in cognitive functioning.

Considering a new population of patients

Patients experiencing symptoms of Long Covid may require support through multifaceted pain management approaches that focus on enhancing the patient's functional abilities and overall quality of life, and that consider the biological, psychological and social aspects of their lives.

“I had noticed that many patients were presenting with new onset pain symptoms that were similar to fibromyalgia after having contracted COVID-19. Given that so much was unknown about COVID, I was curious whether having the virus directly led to the development of fibromyalgia or if a new pain disorder had emerged with similar features,” Dr. Given said.

The case study asks healthcare providers to adapt practices developed for fibromyalgia patients into practices for Long Covid fibromyalgia patients, and suggests a biopsychosocial framework for clinicians to consider when supporting this frequently marginalized population.

“This patient community encounters significant stigma and barriers to their care, and enhancing their quality of life through an interdisciplinary pain management program provides one example of how supportive care can be delivered,” Harrison said.

The study, “Biopsychosocial Management of Long Covid Fibromyalgia,” is particularly relevant to healthcare professionals working on interdisciplinary teams to treat patients with chronic pain, but JIPC covers the spectrum of clinical research. Pleased that their study was selected for publication, Given and Harrison felt JIPC aligned with the work they were doing in a medical setting with a focus on mind-body problems.

“ JIPC is positioned to disseminate clinically useful information and impact how providers interact with their patients today,” Harrison said. “Just as an important advantage of integrated care is to increase access to collaborative management of complex medical conditions, JIPC can highlight the important work of diverse integrated care providers and really move the needle on improving integrated care practices.”

The authors hope that their study will provide a useful review of best practices for screening, diagnosing and treating patients with Long Covid fibromyalgia—especially, as Harrison said, those “who may be feeling misunderstood and lost in a healthcare system that has struggled to widely implement biopsychosocial pain management services for patients with chronic pain syndromes.”

The mentorship experience amplified Harrison's interest in clinical health psychology and helped him to develop a vision for how clinical care can be influenced by asking research questions and sharing lessons learned from clinical experiences.

“Dr. Given not only provided opportunities to help me develop important competencies in psychological assessment but regularly encouraged me to incorporate new research into the experience,” Harrison said. “He encouraged me to think more deeply about the patients we were working with to consider how each patient's medical, psychological and social context interacted with their illness experience.”
